GLP-1 medications have significantly changed the landscape of medical weight loss. The results can be impressive. But while most conversations focus on metabolic health and body composition, there is another important consideration: the changes to your skin.

When weight loss happens, especially in the face, skin has to adapt. Fat loss doesn’t only change contours — it changes the structural support beneath the skin. If that support shifts faster than collagen and elastin can adjust, firmness can decline.

“My goal is to ensure patients feel confident not just in their weight loss, but in their overall appearance,” says Brittney Brasington, MSN, ARNP, FNP-C. “When patients are proactive, we can preserve skin quality and help their results look natural, lifted, and balanced.”

The good news is that with the right strategy, skin can be supported through the process.

Why Early Intervention and A Comprehensive Approach is Essential

Facial fat provides natural cushioning and structural support. As that volume decreases, the skin loses some of its underlying framework. “Weight loss can reduce structural support beneath the skin, so reinforcing collagen early is key,” says Brasington. If collagen and elastin are strong, the skin maintains its integrity. If they are already compromised, laxity becomes more visible.

During weight loss, we commonly see:

  • Increased visibility of fine lines and folds
  • Skin that appears thinner or less dense
  • More noticeable laxity along the jawline and cheeks
  • Reduced protein intake impacting collagen production
  • Increased oxidative stress affecting overall skin quality

This is not simply about sagging. It is about structural resilience. Collagen provides strength and density. Elastin provides recoil and flexibility. When weight loss occurs, the skin remodeling process can struggle to keep pace — especially if nutritional intake shifts at the same time.

When to Begin Supporting the Skin

Ideally, skin support begins at the same time GLP-1 therapy begins. “I encourage my patients to support their skin at the same time they begin treatment,” says Brasington. Collagen remodeling starts early in weight reduction. Waiting until laxity is visible means attempting to rebuild rather than preserve. From a professional standpoint, it is always easier to maintain dermal density than to restore it later.

A practical approach includes starting within the first few weeks of therapy, maintaining consistency throughout active weight loss, and continuing through the stabilization phase. Early intervention helps protect long-term firmness.

Why Peptides Are Especially Relevant

GLP-1 medications are peptides that work through signaling pathways within the body. Injectable treatments such as neuromodulators and biostimulators are often enhanced by peptide-driven communication.

“Considering the diverse benefits peptides provide, pairing topical peptide support with GLP-1 medications promotes healthy and resilient skin” says Brasington. “These ingredients help signal the skin to maintain collagen production, strengthen the barrier, and improve resilience during weight loss. It’s a proactive approach that protects skin quality, not just weight loss results.”

Topical peptides operate similarly at the skin level. They help stimulate fibroblasts, support collagen and elastin production, strengthen the skin barrier, and reduce oxidative stress. In simple terms, peptides send instructions to the skin to reinforce and repair itself.

When internal peptide therapy is paired with topical peptide support, you create layered signaling. You are supporting structural integrity both from within and at the surface — reinforcing collagen stimulation, maintaining barrier health, and preserving firmness during metabolic change.

A Foundational At-Home Regimen

This does not need to be complicated. It does need to be intentional and consistent. “HydroPeptide Power Serum helps stimulate collagen activity and improve visible skin density,” advises Brasington, “while Reactivate Rx Moisturizer delivers advanced amino acid support to strengthen and reinforce the skin’s structure. This layered approach helps patients maintain smoother, firmer, healthier-looking skin during weight loss.”

Here is her recommended daily routine:

Morning: cleanse, apply Power Serum, and follow with moisturizer and SPF

Evening: cleanse, apply Power Serum again, and finish with Collagen Reactivate PM. Power Serum delivers a concentrated blend of peptides designed to support dermal structure and soften the appearance of expression lines. As facial volume decreases, lines can appear more pronounced; strengthening the underlying matrix helps offset that effect.

Collagen Reactivate PM focuses on stimulating collagen production and supporting firmness and density overnight. During active weight loss, this type of daily reinforcement becomes especially important.

Together, these formulas provide foundational collagen support while the body is undergoing change.

In-Office Treatment Pairings

For most patients, topical support can be enhanced with professional treatments. This is not about overcorrection — it is about layered support.

Biostimulatory injectables can encourage gradual collagen production beneath the surface. Radiofrequency microneedling supports tightening while stimulating collagen remodeling. Ultrasound-based tightening treatments can target deeper structural layers. Strategic filler placement may restore balance in areas where volume loss is significant. When thoughtfully combined, these treatments work alongside topical peptides and internal metabolic therapy to preserve facial structure in a natural, proportional way.

Maintenance After Goal Weight

Most conversations focus on active weight loss, but skin remodeling does not stop once the scale stabilizes.

Even after you reach your goal weight, collagen turnover and tissue adaptation continue for months. This is why collagen support should not abruptly end when weight loss slows. The skin is still recalibrating.

The shift is simply from “support mode” to “maintenance mode.”

This may mean continuing daily peptide support, maintaining consistent SPF use, and scheduling periodic collagen-stimulating treatments as needed. The intensity may change, but structural care remains essential.

Long-term thinking protects long-term results.

The Bigger Picture

GLP-1 therapy can be transformative. But transformation should include protecting the quality of your skin. When collagen production is supported early, barrier health is reinforced, and oxidative stress is managed, the skin is far more likely to remain firm, lifted, and resilient throughout the process.

“As a nurse practitioner who works in a practice that prescribes and manages GLP-1 therapy, I see incredible transformations—but I also see how quickly the skin has to adapt,” concludes Brasington. “When facial volume decreases, collagen and elastin need support to maintain firmness and structure. Starting targeted peptide skincare early helps preserve the integrity of the skin so patients look as strong and healthy as they feel.”

GLP-1s incorporate the use of peptides, which is why peptide skincare is the perfect pairing.

If you're starting GLP-1s, start using peptides at the same time, or even before.

You can supplement your skin support with professional treatments like injectables and ultrasound-based therapies.
